Again, I don't think it's a uniquely startup or tech problem. It is evident in many industries that are performance-driven and well-compensated--i.e., finance, law, etc. The common ingredients are poor, ineffective leadership (which breeds fiefdoms), a performance-driven culture (which excuses bad actors that are top performers) and ineffective organization design (which shields accountability).
