A 10-month-old Swedish baby girl developed a “micropenis” after cuddling on her dad’s chest. The father had been using testosterone as a treatment for decreased sexual drive, and unwittingly exposing his baby.

Fortunately it is reversible. Once discovered, the father switched to another form of treatment, as her genitalia shrank back to normal proportions.

The term “micropenis” here is being used by newspapers colloquially, but it is not the scientifically correct term (rather describes an abnormally underdeveloped penis in a male), the right scientific term is clitoromegaly (abnormally enlarged clitoris)

This is a typical problem with parents of infants using testosterone or other hormone topicals without paying attention to potential contact with their children as reported by one of the doctors responding to this case.

A Stanford Case Report from 2021, reported on a very similar case of a 10-months baby girl who got exposed to topical testosterone used by her father to treat a case of low testosterone.

Blood testing revealed the infant had high levels of testosterone and dehydrototestosterone in her blood  (826ng/dL and 851ng/dL 80x and 170x times the norm for her age and sex, levels in the higher range for grown up men).

At 6-weeks follow-up, after the father had stopped his testosterone therapy, the infant's testosterone was back to normal for her age/sex. 

Baby and toddler boys too are at risk of premature puberty if exposed to such high levels of testosterone. A case report on a 10-month old baby boy getting similar testosterone exposure also reported signs of  extremely premature puberty (precocious puberty), which reversed once the father switched his testosterone therapy from topical gel to buccal tablets.

Barechest cuddling with infants is recommended, as it fosters feelings of bonding and safety as the infants can feel the body warmth of a parent, and hear their heartbeats. 

However, parents should be extra careful with skin contact to their infants and children when using any topical hormones, preferably replace with injectable or oral forms if possible.

The FDA has a mandatory clear warning for testosterone gels causing potential secondary exposure in children, sadly the label sometimes goes unnoticed by partners risking exposure and serious complications in their infants and children.

Bottom line:
If you are a parent of infants or toddlers using topical testosterone (or topical female hormones) here is a safety checklist:

• Rub in, air-dry
• Wash hands
• Cover site 4-6 h
• No skin-to-skin till then
