The reason Ralph doesn't drink during the beer competition is that it's a team event. Shortly before this clip (see below), two team captains are assigned, and they each pick people for their team. No-one picks Ralph. So Ralph is on no team, nor is Scottish guy. Then the drinking competition begins, and with Ralph unable to drink anything because he's not on anyone's team he has to stand there and get beer cucked while everyone else chugs beers and gets blackout drunk. When he's going "OH GOD, OH MY GOD" while they're drinking, it's as he finally starts to understand that he is watching them drink ALL of the beer.
