BCBS is the biggest offender. They also control access to several pediatric Medicaid plans in SC and were the ONLY company in the state plans under Obamacare. However, aetna is publicly traded. 

Insurance companies also feed themselves. Aetna is also CVS / Caremark. How that is not a federal kickback, I don’t know. But it is unethical.
