The incident occurred in a five-story commercial building. The accident took place in the mezzanine area between the first and second floors. According to an initial investigation, the victim and a colleague had taken the elevator to retrieve items from an upper floor. On the way down, the elevator stopped between the second and first floors. The co-worker was able to climb out to the second floor and lowered a ladder down to help.
