"Mt Goxs 2014 hack is still one of the most costly crypto incidents ever. At its core was a failure of code security. Mt Gox stored hot wallet private keys on an internet-facing server and the exchange tracked user balances without properly reconciling them against the Bitcoin blockchain. Once an attacker copied the hot wallet file they were able to intermittently drain funds undetected for years. Goxs broken accounting system continued to falsely report customer balances as whole and fully backed"
