"as I look at the health insurance premium spikes for 2026 and how uneven they are - I have to remind people that one of the core problems with how US health insurance structured is we have the double problem of 1) people in many different risk pools (e.g. each employer is a different risk pool) 2) each of those risk pools has different rules around how they're allowed to underwrite risk the end result of this is that healthier groups will increasingly exit any combined risk pool for underwriting that favors them and will leave less healthy groups who then get much higher prices to give a"

"I think you have to choose one path and stick to it 1) individual exchanges with community rating rules and you outright ban employer sponsored healthcare or make it incredibly unattractive by removing the tax-benefitted dollars 2) singapore style mandated HSA accounts with catastrophic public option and private options 3) medicare for all if you're fine with the tradeoffs that brings key though is that everyone has to be in the same risk pool with the same rules of underwriting not this in-between state"
