"The Zen of 3D Wiring ๐Ÿง˜๐Ÿปโ™‚ An effortlessly profound answer from sensei @Laserrman Funny and wise. ๐Ÿ˜‚ ๐Ÿ‘‡๐Ÿผ BTW rotating (the wire) clockwise worked wonderfully for this RCA today. #HDR + 3D wiring for the win ๐Ÿ™Œ๐Ÿผ"

"Interesting quandry This LM quadrification can be broken down into the following equation (where vs = bifurcation): LM = (LAD vs D1) vs. ramus vs. LCX You thus have THREE bifurcations to resolve. One possible sequence: X. Wire X if not all X branches X. LM CBA then IVUS X. Sequential KBI X. Provisional 1-stent for LAD/D1 with JSBT (optional: use DCB for D1) X. Culotte for LAD vs. ramus X. Use JSBT to protect LCX if able to wire Of course CABG is an excellent choice if you can stabilize patient and make emergent CABG non-emergent"

"@mornei2011 @MPAOSS @OjedaOjeda18 @PCRonline @mirvatalasnag @mmamas1973 @jedicath @rotamonster @JuradoRomanAl @Laserrman @crfheart Very nicely done Neisser ๐Ÿ‘Œ The entire intervention is a classy affair. However two things stood out for me: 1) The mid LAD bridge became more apparent post revasc. and 2) The LCX responded very nicely to Rota + DCB"

"As @drrkhyd points out our use of DPDs (as we use to call them) is declining. We dont intervene upon grafts as often. When we do often times DPDs are not needed - so it ends up being operatorss choice. But the one place I use the Spider wire is in CLI for LE revasc. if using atherectomy"

"3D wiring vs. TDADR (or any ADR) Here is a random doodle from some weeks ago when I was thinking hard about 3D wiring. I am a visual learner and my phone is full of these proof of concept pictures. This is my understanding of the role of 3DW vs. ADR in #CTOPCI. ๐Ÿค”"

"#HDR for In-stent CTOs (IS-CTO) ๐Ÿงต Here is the case (and some thoughts) I shared at #CanCTO2025. XX year old with post-CABG angina. Known to have X failed stents and a single LIMA to distal LAD graft"

"Wire choice in #HDR. A common question is which wire to use after the microinjection. We recommend using a soft or medium weight polymer jacketed wire aka PJW. The X common choices are: XTR Gladius MG and Sion Black"

"I saw many #HDR injections this week but this was by far my favorite one ๐Ÿ˜ What is going on here BTW @TDonisan @rajivxgulati and their colleagues have already published this case. It makes for very enjoyable reading ๐Ÿ‘๐Ÿผ"

"Thank you both. I agree completely. Our practice has been #contrast1st with retro or ADR bailout for the past X years. This diagram came out of my discussions here and a review of the literature. It suggests that 3D wiring does have a role - but only at the distal cap. Addition of IVUS makes it even less necessary to use a pure 3DW technique. ๐Ÿค”"

It has a nice shape but it requires a certain anatomy - incl. a smallish cusp. For flush occlusions in a widened root I have had good success with JR4 and/or AL guides. So I just stick with those and leave all the drama for the vessel prep ๐Ÿ˜‚"

"Hany here is my estimate of the FFR in each vessel: distal RCA - XXXX LCX beyond the ISR - XXXX and LAD XXXX. Of course I could be wrong ๐Ÿ˜‚ The point is that all the areas may be involved. This is the cardiac equivalent of the Murder On The Orient Express Personally I would treat the LCX with DCBs now followed by the RCA CTO PCI with DES in a few weeks. I would use IVUS of course and optimize the ๐Ÿ’Š. CABG may be a reasonable choice given the patients age"

"Late Night Thoughts On 3D Wiring Does #3DW have a role in the post #HDR era Yes it does Some useful applications: dTL entry post #HDR stain guided wiring in #CASE re-puncture of prox. cap after failed #HDR IP re-entry from a recovered SB IP navigation round Ca"

"@BarberoUmberto @BotPci @RadialFirstBot @OptimaCTO @Medtronic @MDTVascular @CoronaryConnect @Cardiac_Wire @SCAI @Cardiotweet Nice. Very detailed instructions. I also preach the virtues of a support wire though I have seen one case where a colleague implanted the stent on tge buddy wire thus trapping the filter behind it ๐Ÿ˜ณ"
